Plot the tablet as a rung. Ask what daily total the prescriber is climbing toward before you refill a 600 mg bottle.
Photographers like the scored 600 mg tablet. It looks like a dose. On the PHN schedule it is often a piece of a three-times-daily total. Day 2 uses 600 mg as a whole-day amount split twice. Later, 600 mg three times daily is 1800 mg, the dose that carried most of the pain evidence.
Reviews that treat one 600 mg swallow as a finished nerve-pain plan skip the titration. Dizziness and somnolence rise as the daily total rises. Starting at 1800 mg on a Monday because a review praised 600 mg tablets is how people fall on the stairs.
Food barely changes absorption. An aluminum-magnesium antacid cuts bioavailability by about 20 percent. Wait at least 2 hours after that antacid. If you split a scored 600 mg tablet, take the leftover half as the next dose and throw unused halves away after 28 days.
What the postherpetic neuralgia trials actually measured
Pain that stayed more than 3 months after a shingles rash healed was the entry ticket. People scored pain from 0 to 10 each day. A baseline mean of at least 4 was required. Combined baseline mean was 6.4. That is lingering zoster pain, not a new backache and not a first week of tingling after a disc.
Each study ran a 7- or 8-week double-blind stretch: 3 or 4 weeks of titration, then 4 weeks at a fixed dose. Efficacy showed across 1800 to 3600 mg a day. Extra benefit above 1800 mg did not appear in those studies. Some people cannot reach 1800 mg because of dizziness or sleepiness. A lower tolerated dose is still a dose. It is not a failed personality.
Immediate-release gabapentin is also labelled as add-on therapy for partial-onset seizures from age 3. Restless-legs and once-daily PHN products are different brands with different schedules. A review that mixes Horizant, Gralise, and Neurontin as one pill is reviewing a mash-up the FDA does not treat as interchangeable.
Reviews that skip dizziness, edema, and driving
In the PHN trials, dizziness hit 28.0 percent on gabapentin versus 7.5 percent on placebo. Somnolence hit 21.4 percent versus 5.3 percent. Peripheral edema hit 8.3 percent versus 2.2 percent. Those are the numbers a review should put next to any five-star tablet photo.
Do not drive or run machinery until you know how the current daily total sits in your head. The first week of a climb is a poor week to test a new highway commute. Alcohol and other sedatives stack. Falls in older adults are the practical harm, not a poetic fog.
Suicidal thoughts and behavior are a class warning for antiepileptic drugs, including gabapentin. New mood change, even on a 'just for nerve pain' script, is a same-week call. Gabapentin leaves in the urine unchanged. Half-life is about 5 to 7 hours when kidneys work. When creatinine clearance falls under 30 mL/min, half-life can stretch toward 52 hours. Older adults often look like they have 'normal' serum creatinine and still need the renal row.
From age 12, the table runs by creatinine clearance. At 60 mL/min or higher the daily range is 900 to 3600 mg in three doses. Between 30 and 59 mL/min the range is 400 to 1400 mg in two doses. Between 15 and 29 mL/min it is 200 to 700 mg once daily. At 15 mL/min the range is 100 to 300 mg once daily, and lower clearance scales that down.
After a 4-hour dialysis run, a supplemental gabapentin dose sits on top of the maintenance row. Children under 12 with poor kidney function were not studied for that table. A 600 mg tablet three times a day in a person on dialysis is not a generic-review flex. It is a math error.
Climbing to 1800 mg/day without rushing

Three-times-daily spacing matters for the seizure indication, where the gap should not exceed 12 hours. For PHN the same habit keeps levels from sagging overnight. Missing the midday swallow and doubling at night is how somnolence stacks.
Bioavailability falls as the daily total climbs. About 60 percent at 900 mg a day, about 47 percent at 1200, about 34 percent at 2400, about 33 percent at 3600, about 27 percent at 4800, each in three divided doses. That is one reason piling past 1800 mg disappointed the PHN statisticians.
Why a taper is written into the label
If the dose is cut, stopped, or swapped, the Neurontin label wants that change done gradually over a minimum of 1 week. A longer taper is allowed when the prescriber judges it safer. Abrupt stops are how withdrawal symptoms and, in people who also have seizures, breakthrough seizures show up.
Reviews that say just quit when the pain fades skip that line. Even a 600 mg three-times-daily habit is enough drug to respect on the way down. A weekend without tablets because the bottle ran out is an unplanned taper. Call for a refill before the last day.
Anxiety, insomnia, nausea, pain rebound, and sweating get reported when gabapentinoids vanish overnight. Those are not proof the drug failed. They are proof the descent was too steep. Plan the week with the same clinician who planned the climb.
A 600 mg tablet three times daily is 1800 mg. That sentence is the whole review-photo problem. People see 600 on the score line and think they have reached the study dose after one swallow. They have reached one third of the usual PHN target, or the whole of Day 2, depending on the clock. Write the daily total on the vial cap if the math keeps slipping.
Epilepsy add-on dosing in people 12 and older starts at 300 mg three times daily and may run 300 to 600 mg three times daily. Long-term work tolerated 2400 mg a day in many people and 3600 mg a day in a smaller set. Those seizure numbers are not a PHN shopping list. If seizures are the reason for the bottle, the 12-hour gap rule is not optional.
What generic gabapentin is not interchangeable with
Immediate-release gabapentin capsules and tablets are the Neurontin line. Gralise is a once-daily gastroretentive tablet for PHN only. Horizant is gabapentin enacarbil, a prodrug with its own restless-legs and PHN schedules. Switching among those three by milligram face value is not a labelled substitution.
Pregabalin is a related gabapentinoid with its own titration and a controlled-substance schedule in the United States. A review that says they are the same pill with a different sticker is wrong. Cost conversations belong at the window for the exact NDC you were prescribed.
Generic IR gabapentin is usually widely stocked. This briefing will not invent a 600 mg cash figure. Ask the pharmacy to quote today's cash, today's coupon, and today's insurance copay. A mail cart without a prescription is not a review, and it is not a fill.
Close the file, do not slam the bottle
Issue the 600 mg tablet as a counted piece of an 1800 mg day, or as a Day-2 total, or as a renal-adjusted piece. Do not issue it as a personality. If the pain never was postherpetic neuralgia, send the question back to the examiner who can name the nerve.
